Are these masks niosh approved?

Guys,  This company is getting a few bad reviews that they don't deserve.  NIOSH never approves KN95s nor KF94s.  Also, for the person that thought these were made in the USA, the information where all the pictures and most of the information has the name of the Chinese manufacturing company.  I'm sure that much of the time, the company Amazon deals with is the distributor.   Another thing that often happens, is when something doesn't fit but the dimensions were given, that is not the fault of the vendor.  I am sure that their reviews reflect heavily on their existence with Amazon, and just like everyone else, we all want our income.  If frustration guides our review because it isn't the size you expected or something doesn't fit in your home where you hoped, or a color isn't exactly the same in person, this is the life of on line purchasing.  If we care to go to a store and look around carefully at each item (where we would probably also pay more, it would lessen these situations.

Are the reviews accurate about these running a little smaller? I wear kid size cloth masks, but would like to have some disposable on hand too.

If you wear a kids size mask, this won't work for you. These masks run extremely large. I have tried atleast 20 different brands, never bought one as big as this one. Needless to say, these masks won't work 
 without wearing adjusters on the ear straps, which these don't come with. Pass. Especially at 2.00 a piece.

Are these niosh approved? and if so, what's the approval number?

NIOSH does not approve KN95s, or KF94s.  They do not approve any other that may receive an international approval or certification.

Do these actually provide protection?  Seem too thin to actually be effective

Being better than a regular pleated mask doesn't say much.  I too have wondered about quality of protection because the fact is that these and the KR94s are more comfortable and easy for us to forget we can't forget about protection.  Research says to find the middle ground between comfort and protection because you can get the best of both in the same mask.  So that makes me believe, and this is just speaking about the 3d-fold fish shaped masks only, that the 3M are the absolute best but have the head strips.  The new ones of this shape are not uncomfortable and you certainly feel the thicker softer mask.  I have the 3M 9210+ and the 3M 1870+ which is about the best.  I am thinking of cutting the straps of one and tying them behind my ears to see if that interferes with its integrity.  Then, I thought the KN95 were thinner and the Kf94 the thinnest, but not sure that is the case.  They seem to be about the same.  The Korean standards for the KF94s is more rigid than the Chinese standards for the KN95s but neither are as strictly regulated by the American NIOSH standards.  That tells you a lot.  All research says there are more KN95s that are counterfeit than the Korean mask.  I see that some people go by a picture of a flag and get angry, calling them trash, because they were not made in the US.  The fact is that yes they have been dishonest to attract more of us who feel strongly against imports but keep shopping here.  The truth is that some of what we are wearing now, a lot of food products in our kitchens, a lot of everything we own is Chinese. So that mask may not be trash, and may consider the general graphic of a US flat just a marketing attraction and the mask may be very good.  I try not to judge most of the time too harshly because we know we are ealing with imports and should get really informed before purchasing.   For example, if they say something mesures certain dimensions, and we find the mask too big or small, that is not their fault because we can see in our mind or picture those particular dimensions on our face.  In other our judgment was not good regarding how they dimensions would fit our face, it is not the vendor's fault.  This is like shopping from store windows.  Mistakes are made and we can't run in and try it on.  That is the choice we make.  After a while, you get better at judgment and better as some  vendor pushing too hard or seeing some of the indicators of counterfeit.  I read a lot and watch all of this because i am over 70 and very immunocompromised.  So between the two, I might as well get use to a mask like it is a part of my face. Best of luck
